Dealing with diabetes is very complex, for patients and healthcare providers.

program should also deal with nutrition, weight management, exercise, emotional issues and more. Therefore, at DiabetesAmerica, our patients work with not one, but a whole team of professional whose sole purpose is to provide all the

SPECIALIZED DIABETES MEDICAL CARE
Our highly-trained medical professionals work closely with patients to help control blood glucose levels, treat complications, and promote wellness through individualized care programs.
We use the latest in diabetes research and technology, including insulin pumps (all brands) and training, Byetta, Symlin, and DPP-1V inhibitors.
DiabetesAmerica uses in-house metabolic testing and fundus scans to help identify patients' needs.
PERSONALIZED DIABETES EDUCATION
We provide comprehensive patient education in a setting created exclusively for diabetes care. Our focus
is on providing the tools our patients need to self-manage and become both healthier and more independent.
We are currently using and instructing patients on their insulin pumps and Continuous Glucose Monitoring
Systems, including:
- Omnipod Insulin Delivery System,
- Medtronic Minimed pumps,
- Animas pumps,
- Cozmo pumps,
- Guardian RT, and
- DexCom
NUTRITION COUNSELING / WEIGHT MANAGEMENT
Our dietitians and other diabetes professionals tailor a nutrition plan according to individual food preferences,
and then provide the tools and strategies people need to stay with their plan.
LIFESTYLE INSTRUCTION AND EXERCISE
We help patients integrate regular exercise ad healthy lifestyle habits into their daily routines. These activities become part of their "fitness and exercise" prescription, which is as important to their health as their drug prescriptions.

- 24/7 online access, to physician contact, lifestyle instruction and more
- 24/7 phone line, for counseling support
- Full-service Pharmacy, with free overnight delivery*

*Free overnight delivery available as far west as San Antonio, north to Dallas/Fort Worth, east to Lake Charles, LA and south to Brownsville. Insulin can be delivered overnight only if ordered Monday thru Thursday. No overnight deliveries can arrive on a Saturday or Sunday.

After 4 visits:
- 59% of our patients have A1c values less than 7.0%, and only 9% have A1c values over 9.0%.
- The average A1c value of our patients is 7.0%.
- 62% of our patients have LDL values less than 100 mg/Dl, and only 14% have LDL values greater than 130 mg/Dl.
- 64% of our patients sustain Systolic Blood Pressure less than 130mm/Hg, and only 14% have Systolic Blood Pressure greater than 140 mm/Hg.
- 62% of our patients sustain Diastolic Blood Pressure less than 80 mm/Hg, and only 5% have Diastolic Blood Pressure greater than 90 mm/Hg.
Gestational Diabetes:
In untreated Gestational Diabetes, the risk of fetal macrosomia (birth weight is greater than 4000-4500 g) is
as high as 40%. Our data, based on 68 observations, indicates a rate of fetal macrosomia of less than 10%.
Average birth weight was 3207 g with a standard deviation of 555 g.

GETTING PATIENTS STARTED
Simply complete the appropriate referral form, based on your city
(Houston, Dallas, San Antonio, Corpus Christi)
and fax it to 832-237-3500. From there, we'll contact the patient and schedule their appointment at our nearest
Health Center. We'll keep you updated on the status of your patient. We'll let you know when their appointment
is scheduled, and you'll receive correspondence from our medical team regarding lab results and treatment plan.
DiabetesAmerica Health Centers are open Monday through Friday, 8am to 5pm. DA Health Center in Katy opens early on
Wednesdays and Fridays at 7am.
